You're booked for two Barnstable inspections in the same week. One is a 1988 Colonial in Cotuit on a half-acre near Cotuit Bay. The other is a 1965 ranch in Marstons Mills near a small kettle pond. Both are Town of Barnstable. Both go through one Public Health Division on Main Street in Hyannis. The watershed-permit posture, the sub-embayment overlay, and the agent expectations are not the same.

The Town of Barnstable is a single municipal Board of Health serving seven distinct villages: Barnstable, Centerville, Cotuit, Hyannis, Marstons Mills, Osterville, and West Barnstable.   • Marstons Mills — a mix of Three Bays headwaters and inland kettle ponds; some parcels feed the Mystic Lake / Middle Pond / Hamblin Pond freshwater system that has its own loading concerns.
  • Barnstable village — the north-side village along Cape Cod Bay; less active TMDL pressure, but Barnstable Harbor and Sandy Neck have their own scrutiny.
  • West Barnstable — Cape Cod Bay watershed; the freshest watershed-permit attention is on the Sandy Neck and Marstons-flowing parcels.

Three Bays (Cotuit, Osterville, Marstons Mills inputs), Lewis Bay (Hyannis, southeast Centerville), Centerville River, and Cape Cod Bay are the four main receiving-water buckets. Identify the right one before you write the report.

Why Village Matters at Submission

A Title 5 in Cotuit on a parcel inside the Three Bays sub-embayment requires the Nitrogen Loading Assessment, the watershed compliance notice, and the I/A-at-replacement language under the town's 2023 watershed permit. The same physical inspection on a Hyannis parcel that is sewered to the WWTF does not — the parcel is not on a private septic and Title 5 doesn't apply. Same town BOH, completely different submission package.

The agent reading your Title 5 inspection report maps the address to the village before they read the inspection. Lead with the village name in the cover memo. If the parcel is inside a sub-embayment, name the sub-embayment in the same sentence. "Cotuit, parcel within the Three Bays sub-embayment" gets the inspection routed correctly on intake.

Submission Mechanics

The Barnstable Public Health Division at 200 Main Street in Hyannis accepts Title 5 reports by email at [email protected] and via the town's online permitting portal. The portal is the cleaner path for sale-trigger work because it generates a date-stamped receipt and tracks the case ID. The email path works but the agent will sometimes ask you to re-upload through the portal, costing you a day.

The package the BOH expects:

  • Title 5 inspection report with village named in the property-description block
  • Nitrogen Loading Assessment for any parcel inside a sub-embayment under TMDL

The Three Rejection Patterns

1. Naming the watershed wrong. Cotuit and Osterville parcels feed Three Bays, not "Nantucket Sound generally." Centerville parcels can feed Centerville River or East Bay depending on which side of the village they sit on. The agent will catch this and the inspection comes back. Use MassGIS or the town's parcel overlay; don't guess from the street name.

2. Treating Hyannis as septic when it's sewered. Most of central Hyannis is on the WWTF. If you book a Title 5 for a Hyannis address, confirm with the homeowner and the town sewer map that the parcel actually has a private septic. Inspectors who arrive at a sewered property and write a Title 5 anyway create a confusing record that the BOH has to reconcile.

3. Skipping the site plan attachment. A handwritten sketch on the Title 5 inspection report is acceptable in some Cape towns. Barnstable agents have been pushing for a separate site plan PDF since the 2023 watershed-permit amendment, especially for parcels inside Three Bays. Including it pre-empts the request.

The Failure Mode I Warn About

Don't write "Town of Barnstable" in the property description and stop there. The agent will read the address, identify the village themselves, and route the file — but if the village context is missing from the inspection narrative, the agent has to fill it in from external sources. That triggers a clarification email and adds five to ten business days to the review.
